i. x⁴ + 16 

Index form = x⁴ + 0x³ + 0x² + 0x + 16 

∴ Coefficient form of the polynomial = (1,0,0,0,16) 

ii. m⁵ + 2m² + 3m + 15 

Index form = m⁵ + 0m⁴ + 0m³ + 2m² + 3m + 15 

∴ Coefficient form of the polynomial = (1, 0, 0, 2, 3, 15)
